I’m not sure if you’re wanting an explanation of the name, or what actually happens?

The testes are continuously producing sperm, but if you’re stimulated you’ll also produce the fluid that they mix with to form semen. If you then have no release, you’ve got a build up of fluid that didn’t get released. And it can be a bit uncomfortable.

As for the name – I think the best bet is that blue tends to be synonymous with cold, and someone who’s been blue-balled can be said to suddenly go cold
