They can’t keep it the spacing absolutely the same, but still close enough to look good.
To resize rings, all jewelers do is cut the part of the ring that doesn’t have the gems on it, so the part that’s “inside” your finger. From there they then can either stretch out that gap and fill it in a bit to make it bigger, or pull the two sides over each other to make the ring smaller.
Obviously this oversimplifies, but that is essentially how it works.
