eli5 how do new words form, get approved and get added in the dictionary?

Like this word “rap” no one did rap back then.. This word didn’t exist. Who made this up and how did it get added to dictionary?

Dictionaries are books. They’re written and edited by people. If the people who create a specific dictionary decide a word should be added to (or removed from) their dictionary they just do it. There’s nothing special about dictionaries.
