i was watching How It’s made, and there was a dedicated machine for a particular part of the product something as little as a proprietary bottle cap. and it was a huge machine. Who got the time to sit and invent the machine for that little piece. think of the variety of such designs, so does that mean you need a different new machine for any other special design?

Firstly you learn to make one of your product. This is done using a low volume manufacturing method, like 3d printing, wood working, or highly manual methods. You keep doing this until you have the product looking/behaving exactly how you want it.
Then there is the second part, you redesign the product so that it can be made from a high volume manufacturing process, like vacuum molding, or specialised machinery.
How is a specialised machine worth the money spent paying engineers to design it? Well if you are making a million bottle caps a year and the machine is going to save you $0.01 on every one then that machine is worth $10,000 Per year for the life of the machine. Typically these machines will last a minimum of 10 years, and typically you would save a lot more than $0.01 over making each one by hand.
