Just gonna hit as quick as explanation as I can because this topic can take a full semester to cover.

It has to do with leptin and ghrelin. Ghrelin signals hunger, leptin signals satiety. In normal healthy people, this balance leads to normal eating patterns. As one becomes obese, leptin receptors are desensitized, meaning it takes a progressively larger stimulus for the person to feel full. Think to Austin Powers when Fat Bastard says “I’m fat because I eat, I eat because I’m fat.”

It’s a vicious cycle, but it doesn’t have to be permanent. By focusing on proper diet and increasing physical activity, sensitivity can be restored and that balance can be achieved again.
