eli5 In movies and video games and stuff, characters get stabbed in the gut all the time and this usually causes them to puke up blood instantly. People have told me this is realistic and have tried to explain how it happens but I never get it. How does a stab cause blood to come out of their mouth?

It isn’t realistic at all. It would be very rare for you to get stabbed in a way that causes significant bleeding, and for that blood to end up inside the airways or intestines, which is what it would have to do to get you to throw or cough it up. You might get small amounts, which you would cough up with mucus and saliva, but this wouldn’t look like the pure blood you see in movies. Stab wounds in the major vessels around the lungs/throat/intestines would bleed into the chest, on the ground, or in the abdomen. I have never seen it happen like it does in movies.

Someone stabs me in the gut? My stomach has ruptured and blood is filling it rapidly. You are now regurgitating or puking blood.
Someone stabs me in the chest? A lung is ruptured and/or collapsed, as well as rapidly filling with blood. Your lungs dont like having liquids in them, so youll start coughing up blood.
There are myriad ways it can happen, but it generally goes like this: blood somewhere it shouldnt be is bad. It must be removed.
