Is it a problem in the control room of the TV station? Is it overbooking of commercial spots? Why do I sometimes see a commercial play for about 1 second, and then it cuts out and another compliantly different commercial plays instead?

Hit times being off. Long ago NBC nightly had 1 commercial you could overwrite with a local if you wanted to. The exact hit time is precise, with triggers and sometimes mechanical pre roll on tape machines / switching, it was easy to be off half a second. If you did nothing, the ad on the nbc feed would run.
Live events are more complicated to hit than recorded. You dont want to hear about scte 35/104 triggers and ESAM, but it’s a timing issue when you see the commercial you shouldn’t.
