Surgery in order to lower pitch (voice deepening) of which the vocal folds vibrate is not the first clinical choice. The vocal folds are tiny and delicate structures, roughly 20 mm in males and 15 mm in females.

Thyreoplastic surgery – incision through the thyroid cartilage (lets say Adam’s apple for the time being) can be used in order to make a small cuts on the central part of the vocal folds – which is the main part of the vocal fold that vibrate when we produce our voice (phonation). This small cut can loosen the vocal fold structure which in turn can lower the frequency of which the vocal fold vibrate: lowering the pitch. So yes they would be more loose. Thyreoplastic surgery is however not mainly used for this purpose, typically it’s used as a procedure to alleviate voice dysfunction associated with problems innervating the voice box (dysfunctional nerve signaling to the muscles controlling the voice box) effecting the persons ability to close and/or open the vocal folds.

Thyreoplastic surgery is not first option for lowering voice pitch if that is the desired effect however. Voice therapy targeting a habitual change in your everyday voice production is more desirable since surgery always pose a risk. If you would get thyreoplastic surgery for lowering voice pitch any good otorhinolaryngologist would make sure you get voice therapy pre- and post surgery in order to make sure you are using you voice optimal, otherwise the person risks further harm to the vocal folds in the healing processes.
