If you are in Australia, the difference is that a plastic surgeon has been through specialist training, another 5 years training on top of the standard 6 year medical degree plus internship and residency, including additional exams, so at least 13 years of medical training.

Any doctor with a standard medical degree can immediately call themselves a cosmetic surgeon.
