Eli5: What would be the protocol for the case of an Astronaut suffering a heart attack or similar serious medical condition, (and has it ever happenend before?) on the ISS?

They have medications on the ISS, and doctors on the ground to direct the treatment. Since returning to Earth is highly stressful, that’s not a great option (though a fast-ish return is always possible). Ideally, only very fit and healthy people travel into space.
