Why can some pills be taken on an empty stomach and work while others are advised to be taken with a meal, or they may make you sick or the expected affect may come on stronger?

Some pills are harsh to your throat, so you want to make sure to swallow them “all the way down”. For instance, an antibiotic I was recently on after surgery came with instructions to take it with food and water, but also *not to lie down* immediately afterward, because reflux could put the pill back into the esophagus.
