Eli5: Why does an Appendix get infected and why hasn’t it happened to everybody?

There isn’t one reason but the most common cause is you get a blockage in the lining of the appendix. The blockage causes bacteria to quickly build up & results in infection and appendicitis.
I’m not really sure how to answer your second question. That’s kind of like saying “if falling over can cause someone’s leg to break then why doesn’t everyone break their leg at least once during their life”. There are just too many possible factors to explain why something didn’t happen.
