Eli5- why if you have a blood disease can you not replace your blood?

Like if you have sickle cell or blood cancer or things can you not just like use a dialysis machine to take out your sick blood and do new clean blood?

My BIL had a donor bone marrow transplant from his son. He went from probably having weeks to live to having survived well and is passed his 3rd anniversary.

Random crazy side effect. Some of those stem cells went rogue and ended up becoming hair cells. After years of bald he’s got a nice crop of hair that is the same colour and wave as his son’s hair, not a bit like his original mop.

Exchange transfusions are used for severe sickle cell crises. But it’s not practical to do them all the time and they come with risks.

Bone marrow transplants from a donor are incredibly dangerous and require taking drugs to suppress the immune system which cause many problems.

There is a cure for sickle cell, it involves gene editing to allow for production of normal hemoglobin. It is safe and effective but currently is very expensive. Its one of if not the most expensive therapy in existence right now (with a cost >$1 million per person to cure).
