How are DVD “easter eggs” created?

The DVD menu system is more complex than it seems. You can make buttons/links pretty much anywhere, make links only available at certain times, and act on simple variables. Most easter eggs are presented by abusing these features to hide a piece of video until someone finds the button and presses it.
