They contract with the PBMs and use their negotiated price. They make money by charging a very large processing fee,$5-$10, to the pharmacy. The normal processing fee for a claim is 5-25 cents. It’s not cost effective for pharmacies to accept those cards but are forced due to contracts with the PBMs.

There are so many markups in prescription medicine that the companies are still making a profit selling it with such a markdown. (stupid US medical system.)
