For the same reason that those don’t kill the rest of you. Cancer cells are still part of your body, just running wild. They still appear as “you” to your immune system so don’t get targeted.

Bad luck most of the time cancerous cells appear all the time but sometimes they may slip under the Radar by chance. Ofc if you smoke, you could be producing thousands of cancerous cells a week and thus a few have a larger chance of going unnoticed.
