Basically because a surgeon knows where the major veins and arteries are before going in (roughly) and also knows how to recognize them. Small blood vessels may need to be sutured during the procedure, and blood transfusions can cover for blood loss.

Bottom line is that (outside of trauma cases) the surgeon plans accordingly and knows the lay of the land well in advance.
