How do people get tattoos on their eyes (like what is the process)? How does it effect them afterwards ( like do they have trouble seeing because of the tattoo or is their vision the same)?

I had to stop researching this topic after a few minutes. The process is similar to regular tattoos with needles coated or filed with ink are jabbed into the eye. It is NOT SAFE and generally causes eye problems, damaging the eye and occasionally causing blindness.
Assuming that the pupil area isn’t tattooed, then the vision won’t be directly affected. Plenty of risk for other complications. I don’t get tattoos in general, but I definitely would steer clear of the eye.
