Sex scenes are planned carefully beforehand by the actors. They decides what they feel comfortable with or not on their own and is not pressured to do anything by the director or anyone else. There are a number of things you can wear such as skin colored underwear without visible seams, these can be further edited in the post production to become invisible. And you can cover things up with tape or similar things so that even if you need to see the actor from behind they can be covered up in front. The rest is just well done acting by the professional actors.
Angles, editing, and direction. They are actors after all, they’re paid to make it seem like they are doing something they aren’t actually doing. And this extends to any stunt personnel as well.
It is almost certainly *not* the case that they are having sex or even naked (while next to each other). They’ve gotten very creative with coverups that make it seem like you’re naked but your genitals are actually covered up.
Edit: Yes, I know it has happened occasionally. Which is why I said ” almost certainly” not just “certainly”. The vast, overwhelming majority of movies. This does not happen.
Recently there’s been a development called intimacy coordinators whose job is to figure out how to simulate sex footage without making the actors too uncomfortable.
This can get really complex in something like Euphoria that is so much like p0rn it has clips hosted on those sites.
