The sounds you make are a combination of the vocal cords and the shape of your throat, tongue, nose and mouth. For a fun demo see https://dood.al/pinktrombone/

Part of what happens when you smile is that the pitch being emitted goes a little sharp. One of my chorus teachers used to tell us if we were going flat during a part of the song to smile and it will bring it back towards the tone. There are probably other things that happen to the sound too, and we learn these things while picking up language and correlate it to the expression on a person’s face. Then later when we hear the change in sound we know how the face is most likely to look, even if we can’t see it.

And why do I find it insufferable?

Well…differently shaped mouths obviously change the sound thats coming out of them.
We have experience in listening to ppl talk so we can hear the difference.

Did I understand your question wrong or is that really all you wanted to know? Im a bit putzzled because it’s so self explanatory.
