Generally a drug being used long term is for some condition where it has clear benefits. This gives good rationale to use it in the long run as its benefits outweigh any known long term side effects.

After that there is an established cohort of people using the drug long term, and anyone joining after that cohort can get at least some information on how the drug will affect them in the long run.
