You know how a spring can be compressed and it will, uh, *spring* back and forth? Imagine you had a spring that could do that extremely well without slowing down as fast as a traditional curly spring. It sits inside a mechanical watch and is wound with a handle on the outside of the watch. It is specially designed to always spring back and forth once a second. Simply connect some gears in the right way, and those gears will tick exactly once a second.
