Unit commanders collect the numbers of killed and injured (“casualties” includes both, because either way you’re not combat effective) and pass them up the chain. They are then tallied to arrive at the total. So the squad leader counts their men, passes that up to their commander (more likely to a clerk working on behalf of that commander), who then passes it up again. Eventually, you have total numbers, which is then usually changed to look better or worse than it really is, depending on what kind of reaction they’re hoping for.
