It dilates blood vessels which allows more blood to flow into the penis to maintain an election. It doesn’t actually increase arousal or start an erection, just keeps it going longer and harder.

A penis gets erect because more blood flows into it. Normally, there’s a natural chemical that keeps the blood vessels in the penis relaxed and open so they can fill up.

This chemical naturally gets broken down by an enzyme after a little while. Viagra and similar drugs block this enzyme, so the blood vessels stay open longer –> longer-lasting and stronger erections.

Viagra AKA sildenafil, was originally manufactured as an anti-hypertensive medication but had an unintended side (?bonus) effect that caused dilation of the veins in the penis leading it to become more erect which is what the drug is often used for today.
