I understand how the first 4 quarters are paid out. How do they come up with the 5 quarter?

1st,2nd,3rd,4th and final
If the 4th quarter ends in a tie, then overtime happens. And the winner of the final score pool will be determined by the end score after overtime
If there is a winner after the 4th quarter, that pool winner gets a double prize for having both the final score AND 4th quarter score
