If there are billions of transistors in a CPU, there is no chance that somebody designed every single one of them manually. Is their layout calculated or something?

Short answer is yes. It’s called “digital synthesis.” A computer will place the transistors automatically and wire them up, and make sure some rough functional conditions are met.
