Oh they definitely can and do get tangled.. it happened to me in 2014, it’s called testicular torsion and it is by far the most horrible pain I have ever experienced in my life, and I feel like possibly the worst pain a guy could ever experience, besides perhaps being shot… I legitimately thought I was dying when it happened to me, and it only happened because I was running too fast on the treadmill, and they just got twisted up. Fucked. Absolutely fucked.

Because the balls aren’t really just free-hanging side by side in your scrotum. They are separated by a thin wall called the scrotal septum.

Typically your testes are connected to the scrotum wall by connective tissue too, which prevents them from moving around much. Testicular torsion occurs when a single testicle becomes rotated, compressing the blood vessels supplying the organ. This is much more likely in individuals who have conditions in which this connective tissue is weakened or less developed.

Usually they are “anchored”. They sort of stick to the lining of the scrotum so they move but not too much. Sometimes they arent stuck at the back, this is known as a bell clapper deformity and it is one of the biggest risk factors for testicular torsion
