The usual case is for the right lane to merge into the left lane. There are exceptions, but this is by far the most common method in the USA.

They’re all supposed to have the right lane merge going left (because the driver is on the left and has a better view on that side) but some states still have older designs in use and haven’t upgraded them to modern standards.
