I know O2 has to play into it, so do they get less hot at higher elevations? Or hotter than normal in O2 enriched environments?

You are correct that oxygen plays a role. The package contains iron, salt, and charcoal. Once open to the atmosphere, the iron begins to react with oxygen and oxidize, releasing heat. The higher the concentration of oxygen, the faster that reaction would proceed. If you opened the package in an anoxic environment (one that has no oxygen) than no reaction would happen and you would have room temperature handz.
