It’s when you do various biotech things but in a bit of a DIY/start-up setting, and feel you deserve a more radical-sounding label because of that.
Alternatively, a vaguely defined health guru buzzword.
It doesn’t really mean anything specific. “Hacking” has been corrupted to mean “doing something beneficial easier than expected”. It’s a red flag that something employs psuedoscience or just scams people.
