Mainly digestive/stomach issues, especially GERD. Potentially somewhat elevated blood pressure too, but as long as your consumption is sensible, though, not much else. Coffee is generally considered healthy, assuming it is unadulterated and you aren’t adding too much sugar or anything. One thing to note is that while coffee generally is not carcinogenic and may even reduce risk of certain cancers, this is not true with smokers, I believe. As a bronchodilator, caffeine opens the airways, this may be why smokers who drink coffee are more at risk for lung cancer than smokers who do not drink it.
One thing to note though, it *may* cause mild, chronic dehydration if you dont get adequate water, and since it increases the speed at which things pass through your body (probably other factors involved), you will absorb less nutrients from your food, such as iron. So, you may want to increase nutrient intake, or avoid coffee anywhere near meals, and drink water more often.
Now, as for mental effects, caffeine is a stimulant, so it will increase anxiety in those prone to it. You may also develop insomnia. On the flipside, if you use it regularly you may build up a tolerance and become addicted, in fact I believe it’s the most common drug addiction on Earth. This will mean that you will need it to function, and will be unmotivated, depressed, lethargic and irritable when you aren’t getting enough. One good thing though is coffee is neuroprotective due to caffeine and antioxidants, so “strengthens” the brain and memory and can prevent age related mental decline.
TL;DR: Not many serious downsides on the body, but there are still some to be aware of. It’s actually quite healthy, but caffeine is still an addictive drug, which does mean some side effects on mood and cognition are possible. As with everything, moderation and common sense are key.
