Bad air quality differs depending on context. Given what’s going on right now, you’re talking about smoke from the canadian wildfires?

Basically, the smoke particles get into your lungs and you’re not supposed to have particles in your lungs. Your body does have a mechanism to protect itself from that sort of thing, but there’s so much smoke that it overwhelms that mechanism and the particles end up interfering with lung operations and causing damage. If it’s bad enough or long enough the damage can be permanent.

It’s better to stay inside where you have filters that will take out enough of the particles to reduce the risk of damage.

In the short term, the greatest risk is for people that already have respiratory problems or asthma. Anything that lessens the oxygen they get or triggers a response from their lungs could cause a series of more serious medical problems.

In the long term, exposure to smoke and smog can cause the development of asthma and damage to lungs. There are also numerous other problems which can be hard to predict. But basically breathing anything other than normal unaltered air for any amount of time is not good for you.
