Have you ever eaten a bunch vitamins on an empty stomach?

It affects how the medicine is absorbed in the stomach!

If there is nothing else in there, the medicine will be absorbed faster than otherwise, which can cause issues, depending on the medicine.

Depending on the medicine in question and how strong the stomach is of the person taking it, it may not matter.

But as a general rule of thumb, having food in the stomach when you take a pill makes it so you absorb the medicine in that pill more slowly, since it is also being absorbed by the food in your stomach. And for large and very solid pills, it helps keep the stomach from being irritated buy the pill being ground up .

It can hurt your stomach lining or make your acids out of control and possibly make you puke

In addition to other responses, some medicines are fat soluble so need food in there to go into

There was a medicine my dr gave me for my acne, she said now whatever you do, dont eat when you take this. I was like okay, gotcha! I would take it when I woke up and wait to lunch to eat but I would always have horrible stomachs pains soon after the pill. Well I stuck with it and a few weeks later I went to the follow up and i told her my stomach hurt and i did what she said and didn’t eat. she was like no i told you to make sure to eat with it. No i dont think she did but that’s why my stomach was hurting. Didn’t hurt at all afterwards when i started eating in the morning with my pill.
