The short answer is, no one is really sure.

Genetic studies have shown that genetics does play a role, explaining why it can run in families. The current thinking is that it’s a signalling issue between the brain knowing what it wants to say and telling the muscles themselves what to do.

Thought this [NPR interview](https://www.npr.org/templates/story/story.php?storyId=91284151) was a cool high level explanation. It could be stress induced, it could be we fixate on the wrong word, our memory could be temporarily blocked but an association or hint could unlock it. Lots of papers and research on this phenomenon called “tip of the tongue phenomenon” or lethologica.
