Specifically when they pass out they are unable to breath from such devices.
If you do not already have a breathing devices on you, there will float in some water when equipping it. The water has to be pushed out so that you don’t accidentally breath it in. Depending on the device you need to exhale in a quick burst to achieve that. When diving it can be favorable to breath out some air to reduce your boyancy. So they might not have enough air left to push the water out. Even if they would, it could be the better idea to just surface rather than wasting time underwater to equip something. It’s especially bad if you start to panic.
