Why does your brain know where your hands are but not your internal organs like liver and kidneys etc?

The concise answer: many people died because they misplaced their hands or feet. Not enough people died because they misplaced their liver or kidneys.

Not knowing where our internal organs are was irrelevant to reproductive capacity.
