When you cough, it flexes (squeezes) all your core muscles. That raises your blood pressure. Spiking blood pressure can cause headaches because it physically enlarges blood vessels, including the ones in your head. The vessels expanding due to the higher pressure inside them presses on the surrounding tissue and nerve endings, which can cause a headache.
