It has mostly to do with how the medicine is absorbed.

Going through the mouth is a great way to get into the body, but you have to deal with the digestive system.

Whether it’s through the butt, or the mouth, the intestines are a fantastic way to get something into the blood stream. Through the butt avoids the digestive system and both have different effects such as rate of absorption. Injecting something straight into the blood stream is basically an all-at-once deal, where something absorbed via the skin, or intestines have an over-time effect.

All sorts of logic, but it mostly boils down how quickly does the medicine need to be delivered, if not quickly, does the medicine need to be delivered over a long period of time to be effective, and does the digestive system harm the medicine in some way.
