No idea about the tar, but nicotine doesn’t get stuck in your lungs. Its a chemical (actually a natural pesticide) that is absorbed into your blood stream via your lungs.
Contrary to the common messaging nicotine in and of itself in the doses contained in cigarettes isn’t really harmful. It is addictive, extremely so. And other things in cigarettes are extremely harmful.
Do you have a source for this use? As I [read it](https://www.ncbi.nlm.nih.gov/pmc/articles/PMC5506114/), whole lung lavage is a treatment for [pulmonary alveolar proteinosis](https://rarediseases.org/rare-diseases/pulmonary-alveolar-proteinosis/) a rare disease in which proteins build up inside the lungs. While smoking is thought to be one possible trigger for this disease, I can find no reference to its use to reverse any of the common effects of smoking.
