Can intense exercise damage blood vessels in the head?

Not just exercise, intense exercise, like 1RM deadlifting with veins popping out your neck feeling like your head will explode

Do blood vessels stay enlarged from this or do they gradually resize according to baseline?

Is this damaging for blood vessels or is it fine?

No, that would be very unlikely to happen. Veins, in particular, are very ‘distensible’, meaning they are sort of floppy, and can dilate to hold more blood. The reason your veins pop out when lifting like this is due to a Valsalva. There is increased pressure in your abdomen/chest, preventing as much blood from getting back to your heart. This is more likely to cause you to pass out, since your heart pumps less blood out when it get less blood in. However, the pressure in the veins is still pretty low, and wouldn’t cause the vessels to burst. Blood vessels can rupture from increased pressure, but this is due to underlying damage to the vessels. This is what happens when in an aneurysm that ruptures.

You can get exertion headaches that are caused by the rapid expansion of the blood vessels in the brain. Its not exactly damage though and certainly isnt permanent
