Sometimes I watch shows related to prison life as well as listening to stories of ex-cons, and one thing that struck me as odd was the protocol of bending and coughing in front of an officer to see of your hiding anything illegal. How does that work? How do the officers know for sure that there actually isn’t anything in there after one cough? If there is stuff in there, how can it come out just by coughing?

You’re not just bending over, you’re also pulling your butt cheeks apart, which helps to loosen the muscles holding your anus shut. Is the object you stuck up there going to come rocketing out? Not unless its absolutely gigantic. But the person watching shouldn’t see any meaningful motion in your anus when you do that.
If someone has something in their rectum, having them bend over, spread their cheeks, and cough will cause their anus to press out a little bit. At that point a more thorough examination of their rectum can be performed.
