There are companies specializing in this sort of thing, and honestly, there’s no magic to it, just a lot of attention to detail. You find out exactly how big the thing is and what it weighs and what kind of vehicle it can be fitted onto. And then you study the exact route you’ll be taking to make sure it’ll fit at every step of the way. You pay close attention to narrow streets, roundabouts, railroad crossings, bridges, and anything else that could pose a problem, and you just make sure to prepare and check everything ahead of time.
In addition to what the others wrote, the heavy duty transport companies have special cars which they use to drive suitable routes and scan them with lasers beforehand for obstacles. Thus, they get a picture which routes are doable and which are not.
