Most services currently use an API and their frontend and backend is split up. The frontend is the website or app the user sees. And it just sends simple requests to the server. The server is the backend, and has a so called API. This API has a set of commands that the frontend can send. So instead of the client doing a lot of work and processing it just sends commands and the servers do the rest. These people that create bot accounts in mass just gutted out the front end to find out what commands are used to create an account, then send these commands a few hundred times to make new accounts and then send the command to make them follow an account.
They go to the sign up page and fill in the username and password and make an account. Just like any other account. Then they do that automatically a thousand times.
