The punch forces the skull back quickly, but the brain is not rigidly attached, so it squishes against the inside of the skull, then rebounds and bounces around a bit. This can cause permanent brain damage but often just temporary defects like confusion/loss of consciousness.

Also a nerve thats located behind your jaw. Get chin checked the right way and the jaw will move and pinch that nerve giving you a k.o.
