How does my car antenna transmit the cellular signal to my phone which is paired to my car via bluetooth?

Your car antenna isn’t involved.

Bluetooth is kinda like low power, short range radio. Your car and phone are using radio waves to communicate. Your phone is sending data like music, whether or not you’re being called, and call audio to your car. Your car is, in turn sending microphone input (if you’re talking on the phone), and other inputs to tell your phone what to do, like skip a song or answer the call.

The reason only one device can be connected at a time is that Bluetooth technology requires both the controller and accessory devices agree on a frequency to listen and transmit on and other devices can’t spontaneously connect and listen in on that frequency.
