How does my car antenna transmit the cellular signal to my phone which is paired to my car via bluetooth?

It doesn’t.
The cell tower transmits directly to your phone, whether you’re in your car or not. Your phone transmits audio and data to/from the car over their Bluetooth link, which is a separate radio connection.
