How does my car antenna transmit the cellular signal to my phone which is paired to my car via bluetooth?

Your car antenna isn’t involved at all. The system still uses your phone as the transceiver just as with any call. It just pipes the call through the speakers and has basic call control. Your car is basically like a giant Bluetooth earpiece.
