eli5: How is it that certain surgeries can take several hours?

I understand that certain surgeries can be very technical and require a lot of skill and even a couple of hours, but I don’t understand what would make a surgery last upwards of 5 hours. That seems like a lot of time for cutting someone open and fixing them up.

It’s like a car mechanic, €20 to change a bulb, thousands to change a camshaft or so. The deeper (or more complex) the mechanic/surgeon has to go, the longer it takes. Sounds dumb but same principle.
