I think its all just your brain processing something, using different means & attention levels, but i can only speak for myself here.

Internal monologues happen in my case when im focused on a line of thinking or problem. As the voice isnt yours anyway (remember youre not making any noise, youre just imaging your own voice), you could use any voice you can imagine. Do give it a try, imagine youre an old man and imagine what that voice sounds like.

My non-audio monologue in contrast is sort of like background processing on a computer. I dont notice it necessarily, but it takes up resources and makes it harder to focus. My wife can usually tell its happening especially when im doing some sort of chore and frown for no reason, then turn to her after a while and say something like “isnt it weird that X…”, effectively give her the output of some thinking ive done.
