Redness can be explained by the engorgement/dilation of blood vessels. This is caused by histamines, a type of hormone. Eyedrops counteract this response using antihistamines, which have a similar shape and block out histamine receptors on cells, preventing the histamines from actually working. In turn, the blood vessels return to normal size, and the redness disappears.
